Observation

Based on the staff and parent interviews, specifics could not be found regarding how injuries happened and what staff did to prevent the injuries from happening. Pictures of child's injuries indicated that child obtained multiple bleeding scratches on face and neck as well as two bruised bite marks on his arm. More likely than not, there was lack of adequate supervision at the time of this injury incident.
